The Primary Role of the Community Support Specialist
is to assist individuals in their residences and within the community, promoting independence and fostering engagement in fulfilling activities such as music, art, conversation, and social connections.

This Full-Time role involves a combination of weekday and weekend shifts, including overnight responsibilities.


Position Title: Community Support Specialist

  • Key Responsibilities of the Community Support Specialist include, but are not limited to:
  1. Deliver direct care services while ensuring appropriate supervision in designated risk areas during assigned shifts.
  2. Fulfill all shift-related tasks, including household chores and organization as directed by the Residential Manager or Support Directors.
  3. Complete necessary documentation for site operations (e.g., Communication Logs, Medication Logs, Incident Reports, household funds, resident funds, etc.).
  4. Conduct fire drills and maintain thorough documentation.
  5. Accompany individuals to medical and dental appointments as required.
  6. Report any incidents involving staff or supported individuals to the Residential Manager or Support Directors.
  7. Assist individuals in developing daily living skills.
  8. Provide nutritious meals and snacks, considering dietary needs (e.g., diabetic) and promoting exercise.
  9. Ensure safe food handling and proper storage practices.
  10. Transport and/or assist individuals to appointments, activities, and visitations.
  11. Administer, monitor, and securely store medications as prescribed, ensuring accurate documentation.
  12. Support individuals in areas such as socialization, community resource utilization, vocational skills, and interpersonal development.
  13. Adhere to dietary protocols (e.g., diabetic, eating/drinking/swallowing guidelines) and exercise routines.
  14. Follow hygiene protocols as outlined in the Individual Support Plan (ISP).
  15. Empower individuals to lead fulfilling lives according to their preferences.
  16. Participate in all staff meetings, training sessions, and relevant program meetings (ISP reviews) as assigned.
  17. Complete 24 hours of annual training, including a physical examination every two years and CPR/First Aid training annually.
  18. Engage in all designated training and meetings as directed by Residential Managers, Support Directors, and Quality Enrichment Directors.
  19. Recognize and respond to signs of neglect or abuse.
  20. Maintain effective communication with colleagues to ensure continuity of care and consistency in program plans.
  21. Document and report any concerns regarding neglect or abuse using the incident report form, followed by notifying the Residential Managers and Support Directors.
  22. Ensure timely completion of timesheets for biweekly approval by supervisors.
  23. Perform additional duties as assigned by the Residential Manager or Support Directors.
