- Assist in the creation and execution of Individual Support Plans (ISPs) as needed.
- Provide essential residential support in alignment with assessments and ISPs.
- Guide residents in accomplishing their personal objectives and residential aspirations.
- Implement therapeutic strategies as outlined by professional therapists.
- Maintain awareness of the diagnoses of residents within the facility.
- Encourage the development of a respectful and healthy community culture.
- Support residents in building and maintaining therapeutic relationships.
- Regularly log and submit work hours accurately and punctually.
- Communicate effectively with team members during shift transitions regarding resident status.
- Utilize designated communication channels as instructed.
- Oversee and assist in meal planning and preparation.
- Support residents in completing household maintenance tasks.
- Transport residents to appointments, activities, and community events.
- Facilitate access to recreational and leisure activities, promoting active participation.
- Document and report on resident progress and maintain accurate records.
- Administer prescribed medications after completing the necessary training.
- Respond to medical emergencies as required.
- Identify and report any safety or environmental concerns to management promptly.
- Hold a valid driver's license and ensure safe driving practices.
- Honor and respect the cultural backgrounds and personal choices of residents.
- Employ crisis intervention techniques when necessary.
- Assist residents in maintaining personal hygiene.
- Report incidents to management immediately and complete necessary documentation.
- Participate in all required staff meetings.
- Perform additional duties as assigned by supervisory staff.
- Minimum age of 18 years.
- High School Diploma or equivalent.
- Completion of all mandated training programs.
- Certification in CPR and First Aid is required.
- Subject to a criminal background check.
- Successful completion of a bi-annual physical examination, including a TB test.
