About St. Vincent de Paul CARES

SVdP CARES is a non-profit organization dedicated to ending homelessness by making it rare, brief, and one-time. Guided by "Housing First" principles, all SVdP CARES programs are built on the belief that everyone has the right to live in dignified and affordable housing, regardless of any perceived barriers such as employment history, criminal or credit background, mental health, addiction, or religious practices. SVdP CARES works to rapidly house individuals experiencing homelessness within 30 days and concurrently connects them with resources and stabilization support services to meet their unique needs.

SVdP CARES serves 19 counties across over 7,678 square miles of Florida, including Charlotte, Citrus, Collier, DeSoto, Hardee, Hernando, Highlands, Hillsborough, Lake, Lee, Manatee, Orange, Osceola, Pasco, Pinellas, Polk, Sarasota, Seminole, and Sumter counties. Position Summary

As a Case Manager, you will play a vital role in supporting individuals experiencing homelessness on their journey to stable housing and self-sufficiency. You will work closely with clients to assess their needs, develop personalized service plans, connect them with essential resources, and advocate for their well-being.

Key Responsibilities

  • Conduct comprehensive assessments of client needs, including housing history, support systems, employment status, and any barriers to stability.
  • Develop individualized service plans in collaboration with clients, outlining goals, strategies, and resources to achieve housing stability and self-sufficiency.
  • Connect clients with appropriate community resources, such as housing assistance programs, healthcare services, mental health support, and employment opportunities.
  • Provide ongoing case management support, including monitoring progress, addressing challenges, and making adjustments to service plans as needed.
  • Advocate for clients' needs and rights, ensuring they have access to necessary services and support.
  • Maintain accurate and up-to-date client records, documenting interactions, progress, and any relevant information.

Qualifications

  • Bachelor's degree in social work, psychology, or a related field preferred.
  • Experience working with individuals experiencing homelessness is highly desirable.
  • Strong interpersonal and communication skills, with the ability to build rapport and trust with clients from diverse backgrounds.
  • Excellent problem-solving and decision-making abilities.
  • Ability to work independently and as part of a team in a fast-paced environment.

While performing the duties of this job, the employee is regularly required to sit, speak, and listen. The employee is also required to walk, use hands and fingers to type, operate equipment, and maintain records and notes. Specific vision abilities required include close vision, distance vision, peripheral vision, depth perception, and the ability to adjust focus.

The employee must occasionally lift and/or move up to 40 pounds.

Mental Demands

Must handle new and diverse work problems on a daily basis. Personal maturity is an important attribute. Must be able to resolve problems, handle conflict, and make effective decisions under pressure. Must have the ability to listen objectively to people, perceive the real problem and assist in bringing issues to a successful conclusion. Must relate and interact with volunteers, clients, contractors, visitors, and employees at all levels within the Agency.

The environment will occasionally become noisy due to equipment operations and interactions among clients and staff. There may be the possibility of being exposed to communicable disease, possible exposure to verbal abuse or similar behavior from residents/clients. On an as needed basis, employees may be called upon to work outside of normal business hours.
