KEY RESPONSIBILITIES:

The Housing Support Coordinator is a vital role responsible for aiding individuals with Severe and Persistent Mental Illnesses (SPMI) and/or Co-occurring disabilities in their journey towards independent living within the community. This position may involve case management, connecting clients to community resources, and providing ongoing support to help them maintain the least restrictive level of care.
  • Manage an active caseload of community-based clients, linking them with essential support services for independence
  • Facilitate the service planning process aimed at assisting clients in achieving independent living
  • Conduct assessments and prepare relevant social, financial, or medical history reports as needed
  • Employ de-escalation techniques to ensure client safety, including developing safety plans
  • Arrange and provide transportation for clients to community appointments
  • Build relationships with clients, their families, guardians, significant others, and all service providers involved
  • Perform additional duties as assigned

QUALIFICATIONS:
  • Bachelor's degree in human services or a related field
  • Proficiency in computer applications, including Microsoft Office Suite and Electronic Health Records
  • Preferred experience working with Severe and Persistent Mental Illnesses (SPMI) and/or Co-occurring disabilities
  • Must obtain and maintain CPR and First-Aid Training within 90 days of employment
  • Valid driver's license with current auto insurance coverage required
