Position Overview

The Patient Care Coordinator (PCC)/Nursing Supervisor is tasked with diagnosing and addressing human responses to actual or potential health issues. This role involves various services, including data collection, physical assessments, case-finding, health education, counseling, and the provision of care that supports or restores life and well-being. Under the supervision of the On-Site Director of Nursing (OS-DON), the PCC/Nursing Supervisor is responsible for overseeing and administering nursing care to patients. This includes coordinating activities in collaboration with physicians, administrative staff, pharmacy, clerical personnel, and correctional staff. A comprehensive understanding of nursing principles, grounded in biological, physical, and psychosocial sciences, is essential for this role. The PCC/Nursing Supervisor also applies leadership principles to guide and direct nursing staff effectively.

Key Responsibilities:

  1. Deliver direct patient care as needed, utilizing problem-oriented progress notes aligned with documented issues on the Problem List, organized in the SOAP format.
  2. Coordinate nursing care within the unit by:
  3. Assigning suitable nursing staff for patient care tasks, including admissions, sick calls, treatments, medications, and follow-ups. Any staffing challenges must be communicated to the OS-DON.
  4. Employ therapeutic communication skills and effective interviewing techniques to gather relevant historical information and make necessary referrals. Screen patients with mental health concerns promptly and refer as needed.
  5. Conduct rounds in patient care areas to assess, plan, implement, document, and evaluate nursing care delivery. Assist the OS-DON with staffing requirements and provide guidance to nursing personnel.
  6. Ensure continuity of care across shifts through written and verbal communication, relaying special care needs of patients to the psychiatrist, OS-DON, or On-Island Administrator.
  7. Collaborate with the OS-DON to orient new staff within the unit, providing ongoing evaluations and formal assessments at the end of the probation period.
  8. Offer continuous feedback to staff and conduct annual formal evaluation conferences in collaboration with the OS-DON and/or Deputy Director of Nursing.
  9. Identify staff development needs and devise strategies to address these needs in partnership with available nursing resources.
  10. Facilitate meetings with staff, in collaboration with the OS-DON, to share information, discuss challenges, and foster cooperative professional relationships.
  11. Monitor the quality of patient care through quality assessment and improvement mechanisms.
  12. Maintain ongoing communication and documentation with the OS-DON regarding patient care and unit administration.

Coordinator/Supervisor Duties:

  1. Coordinate the interdisciplinary team to provide comprehensive patient care.
  2. Engage with and uphold standards of nursing practice by applying nursing policies and procedures in accordance with established guidelines.
  3. Identify components of the nursing plan that can be delegated to allied nursing and non-professional personnel, providing necessary teaching, guidance, and supervision.
  4. Assist with the orientation of nursing personnel through participation in the precepting function.
  5. Advise nursing staff and relay information to the OS-DON, ensuring timely documentation as required.
  6. Supervise care provided by LPNs/NAs and undertake additional related responsibilities as necessary.
  7. Administer medications safely and efficiently, adhering to prescribed orders and established standards.
  8. Engage in ongoing monitoring activities, including peer reviews, to ensure quality care delivery and recommend changes where issues arise.
  9. Commit to professional development through participation in evaluation processes, formal education courses, and continuing education programs.
  10. Participate in unit conferences and staff development initiatives.
  11. Engage in projects and research activities that identify patient care needs.
  12. Share knowledge gained from continuing education programs with peers.

  1. Contribute to an environment conducive to meeting patient needs by proactively maintaining supplies and equipment.
  2. Demonstrate awareness of the correctional health environment by adhering to security regulations and managing contraband appropriately.

  5. Recognizing and respecting individuals as unique.
  6. Exhibiting concern for unit and patient needs consistently.

Department Preferences:

  • Show commitment to ongoing professional development through attendance at continuing education programs and participation in professional committees.
  • Possess three years of current professional nursing experience in a correctional setting or three years of psychiatric nursing experience in relevant environments, with crisis intervention experience preferred.
  • Exhibit clinical competence and leadership qualities, including decision-making abilities and skills in problem-solving, communication, and interpersonal relationships.

Qualification Requirements:

  • Must be licensed as a registered professional nurse in accordance with state statutes or eligible to practice nursing as per the State's Board of Nursing.
  • Current Basic Cardiac Life Support certification is required; ACLS certification is preferred.

Minimum Qualifications:

For individuals holding an unrestricted registered professional nursing license prior to July 1, 2020, in New York State or other jurisdictions:

  1. A valid New York State license and current registration to practice as a Registered Professional Nurse in New York State.
  2. Holds or obtains a valid and current certification in Basic Life Support (BLS) through the American Heart Association (AHA) during facility orientation.
  3. Three years of experience as a registered nurse.
