LPN Supervisor for Long Term Care Facility

Position Overview:
As a key member of our nursing team at Charles T. Sitrin Health Care Center, the LPN Supervisor plays a vital role in overseeing the daily operations of our Long Term Care Houses. This full-time position offers a competitive pay rate based on experience and includes a sign-on bonus for new hires.

Responsibilities:
+ Oversee the overall management of the assigned house during your shift, providing leadership and guidance to Nurse Aides.
+ Assign and supervise work tasks for unit Nurse Aides to ensure completion of duties.
+ Communicate essential information to the acting Supervisor for coordination with other healthcare disciplines.
+ Administer and document physician-ordered medications and treatments accurately and in a timely manner.
+ Maintain proper procedures for ordering and receiving prescribed medications.
+ Monitor residents for changes in their health status, documenting findings and reporting concerns to the acting Supervisor.
+ Ensure accurate and timely documentation in electronic medical records in accordance with facility policies.
+ Contribute to the Interdisciplinary Care Plan by providing insights on resident care needs.
+ Adjust work assignments and priorities based on the evolving needs of residents, ensuring high standards of care.
+ Document shift occurrences to facilitate effective communication with the incoming shift.
+ Report any changes in residents' conditions to the Charge Nurse or Supervisor.
+ Collaborate with the healthcare team to inform families, physicians, and administration as necessary.
+ Engage in preventative skin care programs to promote resident health.
+ Uphold the highest ethical standards and adhere to the facility's compliance plan and confidentiality policies.
+ Complete any additional duties as assigned by the shift supervisor.

Qualifications:
+ Must be a graduate of an accredited School of Practical Nursing and possess current licensure as a Licensed Practical Nurse in New York State.
+ Demonstrated leadership skills and effective communication abilities.
+ Competency in job-specific skills, validated through annual assessments.
