Job DetailsClinical Practice Specialist - NP or PA
Monday to Friday
7 AM to 3 PM
Catholic Health Physical Therapy and Rehabilitation provides patients with access to award-winning rehabilitation services in an outpatient setting.
Our facility is recognized for its comprehensive rehabilitation services for both adults and children.
This location offers one of the most extensive services for outpatient rehabilitation available in our region, serving as a premier resource for those in need of speech, occupational, and physical therapy. Patients benefit from convenient access to laboratory testing and a team of specialized physicians.
Requirements:
New York State RN license and Registered NYS Nurse Practitioner license, with adult, family, or geriatric certification. BLS certification is required, along with critical thinking skills and excellent interpersonal and technical abilities.Responsibilities:
Deliver comprehensive care to assigned patients in collaboration with the attending physician. Administer medical interventions as necessary. Monitor laboratory and test results closely. Facilitate appropriate consultations as required. Assist with patient discharges and discharge planning. Oversee patient progress in rehabilitation to ensure safe transitions to home or other facilities. Provide care that respects individual cultural and religious diversities and acknowledges patient rights. Ensure thoroughness in patient care. Apply medical and clinical knowledge, particularly in PM&R, and utilize current best practices. Collaborate effectively with others and build rapport with patients and their families. Uphold patient confidentiality and demonstrate ethical standards in treatment. Commit to ongoing personal development. Maintain clarity and completeness in medical records and documentation.
