Job Summary

We are seeking a highly organized and detail-oriented Patient Care Coordinator to join our team at Link Home Care. As a Patient Care Coordinator, you will play a critical role in facilitating care coordination for our patients, ensuring seamless communication between caregivers and clients.

Key Responsibilities
  • Assign and schedule caregivers for clients, confirming availability and acceptance of assignments.
  • Initiate the intake process, developing patient profiles to identify the best caregiver-patient match.
  • Maintain accurate scheduling records, including visit confirmations using HHA exchange and/or Salesforce.
  • Communicate essential information to caregivers about their patients and notify clients and families of caregiver assignments.
  • Respond to voicemail and email messages in a timely and professional manner.
  • Document all calls and follow proper telephone follow-up procedures.
  • Collaborate with the clinical team to convey reported changes in condition or inquiries requiring clinical direction.
  • Work closely with the HR team to provide caregiver-need information for recruitment purposes.
  • Ensure compliance with all Federal, State, and City regulations, as well as agency Policies and Procedures.
  • Adhere to contractual relationships with CHHA, LTHHCP, MLTC, and other relevant parties.
  • Communicate with Contract case managers/schedulers regarding changes, missed visits, and client concerns.
  • Maintain patient and employee schedules in HHA exchange and Salesforce or other electronic/manual scheduling systems.
  • Participate in on-call service duties as requested, ensuring timely responses to calls.
Requirements
  • HOME CARE experience (preferred)
  • Spanish-speaking skills (preferred)
  • Ability to work independently and within a team
  • Multitasking and prioritization skills, with flexibility in a fast-paced environment
  • Optimistic, energetic, and outgoing personality
  • Strong interpersonal communication skills
  • Excellent customer service skills, with empathy and patience for the elderly population
