As a Case Manager Registered Nurse, you will play a vital role in coordinating care for patients requiring rehabilitative services, ensuring that each patient receives personalized attention and support to achieve optimal outcomes.

About the Role

This position requires a highly motivated and organized registered nurse with excellent communication and interpersonal skills to provide program orientation, coordinate patient education, and ensure implementation of individualized treatment plans. The ideal candidate will have a strong understanding of rehabilitation principles and practices, as well as the ability to work collaboratively with multidisciplinary teams.

Key Responsibilities

  • Provide comprehensive program orientation to patients and families, including information on case management services, rehabilitation philosophy, and discharge criteria.
  • Cordinate patient education and promote active participation in treatment planning and goal setting.
  • Ensure timely and effective implementation of individualized treatment plans that address patient strengths, abilities, and preferences.
  • Document findings from Discharge Planning Evaluations and psychosocial assessments, and communicate social, financial, or discharge needs and preferences of patients and families.

Requirements

  • Minimum 3 years of experience in nursing practice
  • Clinical knowledge, IT skills, effective interpersonal communication, critical thinking, problem-solving, autonomy, innovative thinking, and community resource navigation
  • Bachelor of Science in Nursing or related field; Master's in Nursing preferred
  • Current licensure as a registered nurse with the North Carolina Board of Nursing; eNLC license may apply
