Key Responsibilities:

  • Treatment Planning: Develop and oversee treatment plans as approved by physicians
  • Patient Care Coordination: Coordinate with care teams, including nurses, therapists, and home health aides, to ensure effective patient care
  • Communication: Promote communication between team members, attending physicians, and administrative staff to ensure appropriateness of care
  • Care Coordination: Ensure patient needs are identified and qualified caregivers are assigned to cases within their scope of practice

Requirements:

  • Education: Bachelor of Science degree in Nursing (BSN) preferred
  • Licensure: Licensed Registered Nurse
  • Experience: At least 2 years of clinical home care experience with a minimum of one year in a clinical management role
  • Skills: Excellent verbal and written communication skills, knowledge of state and federal home health agency regulations, and ability to implement clinical policies and procedures

Salary Range: $69,800 - $96,200 per year, depending on qualifications and location. Eligible for bonus incentive plan based on company and/or individual performance.
