**Job Title:** Registered Nurse - Case Manager
**Estimated Salary:** $85,000 - $115,000 per year

Responsibilities:

  • Collaborate with colleagues in the Care Management department to ensure quality-driven, best-practice care across the continuum of hospitalization.
  • Maximize positive financial outcomes for designated patient populations by intervening at key points in the disease process.
  • Provide designated leadership functions for the Care Management Department.

Requirements:

  • Minimum of Five years recent clinical nursing experience in a relevant practice area.
  • RN with Bachelor of Science in Nursing or related field.
  • Current licensure as Registered Nurse with the North Carolina Board of Nursing or a compact state as defined by the North Carolina Board of Nursing.
  • Nationally recognized certification in Case Management (ACM or CCM, or ANA approved certification).
