About the Role

  • The successful candidate will have a valid Texas RN license in good standing and a minimum of 2 years clinical experience in a behavioral healthcare setting, preferably with phone triage, tele-health or case management experience.
  • You will be responsible for conducting intake assessments, developing individualized care plans, and collaborating with interdisciplinary professionals to ensure comprehensive care.
  • A strong understanding of psychiatric nursing principles, procedures, and techniques is essential, as well as knowledge of medical and psychological terminology, medications, and their effects on patients.

Responsibilities

  1. Conduct thorough intake assessments to identify patient needs and develop personalized care plans.
  2. Collaborate with multidisciplinary teams to ensure timely referrals and seamless transitions of care.
  3. Maintain accurate records and documentation of patient treatments, immunizations, laboratory results, and progress notes.
  4. Communicate effectively with patients, families, and healthcare professionals to provide excellent customer service.

Requirements

  • Bachelor's degree in Nursing from an accredited college or university.
  • Valid Texas RN license in good standing.
  • Minimum of 2 years clinical experience in a behavioral healthcare setting.
  • Phone triage, tele-health, or case management experience preferred.
  • Basic teaching and training skills a plus.
