Job Title: RN Homecare Float

Cleveland Clinic Health System is seeking a skilled Registered Nurse (RN) to join our Homecare team in the West of I-77 Service Area. As a Homecare Float RN, you will provide high-quality nursing care to patients in the comfort of their own homes.

Job Summary:

As a Homecare Float RN, you will be responsible for providing assessments, complex nursing treatments, and education to help avoid readmissions to the acute care setting. You will work independently to complete documentation, make calls, attend meetings, coordinate patient care, and arrange visit schedules. If there are no visits, you will monitor quality through chart audits and supervisory visits.

Responsibilities:
  • Provide direct nursing care in accordance with established policies, procedures, and protocols of the agency.
  • Participate in and support quality improvement projects.
  • Participate in departments and committees as assigned.
  • Supervise starts of care, resumptions of care, and routine visits as requested.
  • Perform accurate and complete admission assessments and reassessments within a specified time frame.
  • Assess patient for safety risks, abuse, and neglect.
  • Serve as a primary coordinator of all disciplines for well-coordinated patient care.
  • Assess and document patients' pain using appropriate pain scale and manage patients' pain.
  • Prioritize and document patient problems based on assessed needs and problems in collaboration with patients, family, and healthcare team.
  • Maintain complete and up-to-date documentation and files at all times regarding patient condition and effectiveness and timeliness of care and interventions.
  • Escalate change in patient condition to patient's physician as necessary.
Qualifications:
  • Graduate from an accredited school of professional nursing.
  • Current state licensure as a Registered Nurse (RN).
  • One year of current clinical experience as an RN.
  • One year of experience in an acute medical/surgical or homecare setting.
  • Valid Ohio driver's license and proof of automobile insurance coverage with $100,000/$300,000 coverage.
Preferred Qualifications:
  • Bachelor of science in nursing (BSN).
Physical Requirements:

The ideal candidate will be able to:

  • Requires full range of motion, manual and finger dexterity, and eye-hand coordination.
  • Requires standing and walking for extensive periods of time.
  • Requires corrected hearing and vision to normal range.
  • Requires some exposure to communicable disease or bodily fluids.
Work Environment:

The work environment will be in patients' homes, with occasional visits to the Cleveland Clinic Health System facilities.
