Job Summary

We are seeking a skilled LPN to join our homecare team. As a Licensed Practical Nurse, you will play a vital role in ensuring the continuity of care and coordinating discharge planning for our patients.

Key Responsibilities
  • Provide direct patient care under multiple levels of care, collecting and documenting objective and subjective patient data in accordance with agency policies and procedures.
  • Act as a member of the interdisciplinary team, reporting data to the directing registered nurse and other members of the healthcare team to initiate a plan of care and address changes accurately.
  • Engage and educate potential customers on the benefits and appropriateness of care, ensuring patients and their families have adequate knowledge and understanding of the hospice or homecare benefit.
  • Contribute to patient assessments by collecting data for analysis by a RN, observing patients, and reporting any unusual conditions to professional staff.
  • Administer medications within scope of practice, observe patients for effectiveness, and implement safety measures to prevent adverse reactions.
  • Maintain complete and up-to-date documentation and files regarding patient condition and effectiveness of care.
  • Ensure seamless transitions to the residential setting by coordinating services at time of discharge.
Requirements
  • Graduate of an accredited practical or vocational nursing program.
  • Current Ohio License as a Licensed Practical Nurse required, with M-IV certification preferred.
  • Basic Life Support (BLS) through American Heart Association (AHA).
  • Valid Ohio Driver's license upon hire; per policy 8-175 thereafter.
  • Proof of automobile insurance coverage with $100,000/$300,000 coverage upon hire; per policy 8-175 thereafter.
Work Environment

This role requires the ability to work confidently and accurately in a fast-paced, time-constrained environment. The successful candidate will be able to build and maintain relationships with patients, physicians, community agencies, and institutions while providing excellent service and care.

Physical Requirements

This position requires full range of motion, manual and finger dexterity, and eye-hand coordination. The ability to stand and walk for extensive periods is necessary, as well as corrected hearing and vision to normal range. Some exposure to communicable disease or bodily fluids may be required.
