Job Title: RN Homecare Float

Cleveland Clinic is seeking a skilled Registered Nurse (RN) to join our Homecare Float team. As a Homecare Float RN, you will provide high-quality nursing care to patients in the comfort of their own homes.

Job Summary:

The ideal candidate will have a strong background in nursing, with experience in homecare or acute medical/surgical settings. You will be responsible for providing assessments, complex nursing treatments, and education to help avoid readmissions to the acute care setting.

Responsibilities:
  • Provide direct nursing care to patients in their homes, including assessments, treatments, and education.
  • Work collaboratively with patients, families, and healthcare teams to develop and implement individualized care plans.
  • Monitor patient progress, identify potential issues, and escalate concerns to the patient's physician as needed.
  • Document patient information accurately and thoroughly, including assessments, treatments, and outcomes.
  • Participate in quality improvement initiatives to enhance patient care and outcomes.
Requirements:
  • Graduate from an accredited school of professional nursing.
  • Current state licensure as a Registered Nurse (RN).
  • One year of current clinical experience as an RN, preferably in homecare or acute medical/surgical settings.
  • Valid Ohio driver's license and proof of automobile insurance coverage with $100,000/$300,000 coverage.
Preferred Qualifications:
  • Bachelor of Science in Nursing (BSN).
Physical Requirements:

This role requires full range of motion, manual and finger dexterity, and eye-hand coordination. You will be standing and walking for extensive periods, and may be exposed to communicable disease or bodily fluids.

This is a medium work role, requiring exertion of 20 to 50 pounds of force occasionally, and/or 10 to 25 pounds of force frequently, and/or greater than negligible up to 10 pounds of force constantly to move objects.

Personal Protective Equipment:

You will be required to follow standard precautions using personal protective equipment as needed.
