Job Summary

We are seeking a skilled LPN to join our homecare team. As a LPN Homecare Specialist, you will be responsible for ensuring continuity of care and coordinating discharge planning. You will work closely with our interdisciplinary team to provide direct patient care under multiple levels of care.

Key Responsibilities
  • Collect and document objective and subjective patient data, in accordance with agency policies and procedures.
  • Report data to the directing registered nurse and other members of the health care team, to initiate a plan of care and address changes accurately.
  • Actively engage and educate potential customers to the benefits and appropriateness of care.
  • Ensure patients and their families have adequate knowledge and understanding of the hospice or homecare benefit.
  • Contribute to patient assessment by collecting data for analysis by a RN.
  • Observe patients and report any unusual conditions to professional staff.
  • Implement safety measures within scope of practice.
  • Administer medications within scope of practice and observe patients for effectiveness and adverse reactions.
  • Provide direct patient care, including dressing changes and laboratory tests, as part of the plan of care.
  • Maintain complete and up-to-date documentation and files regarding patient condition and effectiveness of care.
Requirements
  • Graduate of an accredited practical or vocational nursing program.
  • Current Ohio License as a Licensed Practical Nurse required, with M-IV certification preferred.
  • Basic Life Support (BLS) through American Heart Association (AHA).
  • Valid Ohio Driver's license upon hire; per policy 8-175 thereafter.
  • Proof of automobile insurance coverage with $100,000/$300,000 coverage upon hire; per policy 8-175 thereafter.
Work Environment

This position requires the ability to work confidently and accurately in a fast-paced, time-constrained environment. The LPN Homecare Specialist must be able to build and maintain relationships with patients, physicians, community agencies, and institutions while providing excellent service and care.

Physical Requirements

This position requires full range of motion, manual and finger dexterity, and eye-hand coordination. The LPN Homecare Specialist must be able to stand and walk for extensive periods of time, and must be ambulatory and able to drive to patients' homes.
