JOB SUMMARY:
The Care Coordinator is responsible for scheduling, human resources tasks, and various office duties, while also providing field support as a caregiver when necessary. Responsibilities include conducting assessments for new clients in their homes and performing regular supervisory visits. The role involves adjusting care plans, maintaining medication records, and assigning caregivers to clients. The Care Coordinator will manage incoming calls and be available for weekend and after-hours calls as required.

ESSENTIAL FUNCTIONS:
  • Exemplary telephone communication skills.
  • Assists with scheduling responsibilities as directed by supervisors. Previous experience with Wellsys (ClearCare) software is preferred.
  • Provides personal care and supportive services to clients in the field as needed.
  • Participates in weekend on-call scheduling and backup caregiving on a rotational basis, including weeknights if applicable.
  • Supports recruitment efforts, including tracking incoming calls, coordinating training, and managing new hires. Conducts background checks and verifies records for all potential hires.
  • Assists the hiring manager in verifying work references for associates.
  • Manages incoming and outgoing mail.
  • Prepares new hire packets, client care record books, and informational packets.
  • Maintains office supply inventory and places orders as necessary.
  • Handles check deposits for Accounts Payable and assists with general bookkeeping tasks.
  • Prepares medication administration records (MARs) for clients and communicates with healthcare providers.

EDUCATION / SKILLS / ABILITIES / AVAILABILITY:
  • High School diploma or equivalent with two years of business experience.
  • Strong office and computer skills, along with excellent organizational abilities and interpersonal relations.
  • Familiarity with common medical terminology.
  • Able to work independently, demonstrating sound judgment.
  • Prior experience as a caregiver is required.
  • Prior experience in scheduling is preferred.
  • Must possess a valid Driver's License and have access to a vehicle.

WORKING ENVIRONMENT:
  • Operates primarily from the local office.
  • Engages in fieldwork with caregiving responsibilities and client services as needed.
