Job Summary:

The Client Care Coordinator at Almost Family is responsible for managing and coordinating Intakes and assessments of new clients and monitoring visits of current clients to ensure maximum efficiency and quality of care for our clients.

This position requires strong organizational and communication skills, as well as a background in healthcare and customer service.

The Client Care Coordinator will work closely with other departments to ensure the smooth operation of our services in Eugene, Oregon.


Responsibilities:

  • Develop and maintain a Plan of Care based on clients' needs and preferences
  • Coordinate with other departments to ensure proper staffing levels and adequate coverage for client care
  • Communicate with clients and their families to schedule and confirm service start, who will be in the home and provide any necessary information or instructions
  • Utilize scheduling software

Monitor and track Clients Quality Monitoring visits


  • Work closely with Manager to assist in orienting new employees, including scheduling training and orientation sessions
  • Develop and implement strategies to improve efficiency and streamline processes
  • Maintain up-to-date knowledge of company policies, procedures, and regulations related to scheduling and client care
  • Assist the team with scheduling emergencies and address any scheduling conflicts or issues in a timely and professional manner
  • Participate in meetings and collaborate with other departments to improve overall service delivery and client satisfaction.

Requirements:

  • High school diploma or equivalent
  • Minimum of 1 year of experience in healthcare or medical scheduling, preferably in a management or leadership role
  • Proficiency with computers and basic programs like Excel and Word
  • Strong organizational and time-management skills
  • Excellent communication, interpersonal, and leadership skills
  • Ability to work independently and handle multiple tasks with minimal supervision
  • Must be detail-oriented, self-motivated, and adaptable to a fast-paced work environment.
