Patient Care Coordinator

Overview:
vAcute is a 24x7 virtual acute care department that provides coordinated personal, professional, and compassionate service to patients. The team is comprised of navigators, RNs, Advanced Practice clinicians, and a board-certified Emergency Medicine physician. This collaborative group works to take care of patients outside of the hospital setting, in the moment, or helps direct the patient to the most appropriate level of care.
Key Responsibilities:
- Facilitate patient/family experience at Mercy Virtual by providing coordinated personal, professional, and compassionate service.
- Answer telephone calls from patients/family in Mercy Virtual vAcute Program.
- Perform duties and responsibilities in a manner consistent with our mission, values, and Mercy Service Standards.
Qualifications:
- Previous experience in a Medical Office, Emergency Department, or Hospital.
- High school diploma.
- BLS required or within 6 months of hire.
- Critical thinking and problem-solving skills.
- Professional verbal and written communication skills with the ability to work in triage software and electronic medical record.
