About the Role:

We are seeking a highly skilled Care Coordinator to join our team at EmpowerMe Wellness. As a Care Coordinator, you will play a vital role in leading wellness classes and coordinating the care of patients in assigned communities.

Key Responsibilities:

  • Lead and organize wellness classes to promote health and well-being among residents.
  • Assist in schedule management, ensuring seamless delivery of care and wellness programs.
  • Coordinate care by liaising between medical providers, nursing teams, and residents.
  • Obtain consent and all necessary intake paperwork from resident or POA prior to initiation of integrated services.
  • Perform data entry tasks and maintain patient electronic health record.
  • Document residents' medical information, including medical history and health insurance information.
  • Manage all internal and external communications concerning resident health and wellness.
  • Collaborate with the Nurse Practitioner and communicate regularly with the executive director and/or director of nursing regarding any areas of concern or successes.
  • Maintain strong relationships with physicians, care staff, administrative teams, and resident family members.
  • Record patients' vital signs, documenting physicians' notes and changes in medical conditions.
  • Comply with HIPAA and PHI guidelines, ensuring patient/resident confidentiality at all times.

Requirements:

  • High school diploma or equivalent; completion of a Medical Assistant program preferred.
  • Certification as a Medical Assistant (CMA) or Registered Medical Assistant (RMA) preferred.
  • Previous experience working in a geriatric setting or with elderly residents highly desirable.
  • Exceptional communication and interpersonal skills are required along with the heart and passion to work with older adults while providing the best care possible.
  • Must have the ability to remain calm in stressful situations, be flexible, work well with many interruptions, and have exceptional multi-tasking skills.
  • Must exhibit and promote a high level of professionalism, customer service, curiosity, and friendliness in all interactions with employees, residents, and visitors throughout the organization.
  • Strong attention to detail and the ability to multitask in a fast-paced environment.
  • Respect for the principles of patient/resident rights and confidentiality, including adherence to HIPAA regulations.
  • Flexibility to adapt to changing priorities and willingness to work as part of a collaborative team.
  • CPR certification may be required.
  • Proficiency in Internet browsers (e.g. Explorer, Chrome, Fire Fox), as well as advanced knowledge of Microsoft Office programs: Outlook, Excel, Word and Publisher applications.
  • Experience in working with various EMR and medical billing systems.
