Licensed Practical Nurse II

The Licensed Practical Nurse II, working under the supervision of the Nurse Manager or designated Registered Nurse, is responsible for delivering direct patient care to assigned individuals.
This role is integral to unit operations and plays a key part in enhancing the care delivery framework at the departmental level.
The LPN II collaborates with team members to implement patient care strategies, ensuring that all care is provided in a professional, compassionate, and respectful manner. The position requires making safety-oriented decisions while delivering patient care and participating in department-wide initiatives.

- Demonstrate Professional Competence:
- Maintain up-to-date professional skills and knowledge.
- Meet the expectations for a licensed practical role within the department.
- Foster a welcoming and supportive environment for patients.
- Ensure High-Quality Clinical Outcomes:
- Take responsibility for the care of assigned patients under the guidance of the co-assigned RN or physician.
- Document patient data, care provided, and medication administration in accordance with departmental standards.
- Engage in patient education alongside the assigned RN as outlined in the care plan.
- Identify and Address Patient Needs:
- Recognize and communicate patient and family needs to the care team.
- Monitor changes in patient conditions and seek appropriate resources to address concerns.
- Promote Safe Patient Care:
- Apply healthcare practices as directed by nursing educators and under provider supervision.
- Support and implement organizational policies and procedures.
- Work collaboratively with team members to establish priorities and action plans.
Education: Graduate of an accredited Practical Nursing program with a valid LPN license in New York State.
Certifications: BLS certification required.
Physical Requirements:Ability to assist or transfer patients with a maximum effort of 50 lbs. Must possess full range of motion to perform essential job functions, including standing, walking, bending, kneeling, crouching, and reaching frequently during shifts.
Language Proficiency:Ability to read, write, and interpret documents in English, including safety guidelines and procedural manuals. Capable of analyzing and responding to inquiries from various stakeholders.
Mathematical Skills:Ability to perform basic arithmetic operations and calculate proportions, percentages, and ratios.
Reasoning Skills:Ability to solve practical problems and interpret a variety of instructions in different formats.
Working Environment:Potential exposure to biohazardous materials and communicable diseases. May require handling emergency situations and working irregular hours.
