Key Responsibilities
  • Prepare patients for their visits with providers, ensuring a smooth and efficient experience.
  • Assist providers in responding to patient needs related to medications, lab results, procedures, and preventative/health maintenance measures.
  • Perform pre-visit planning, utilizing software tools and chart review to anticipate health maintenance, preventive, and chronic care needs of each day's scheduled patients.
  • Room and prepare patients for provider visits, including vital signs, medication reconciliation, and verification of allergies and social history.
  • Perform in-house labs and procedures, such as EKGs and peak flow meter readings, in accordance with standard nursing practice and health maintenance/chronic care protocols.
  • Prepare lab specimens for sending to the lab and provide age and patient-appropriate screenings, documenting and reporting as necessary.
  • Administer vaccines, injections, and PPDs in accordance with standard nursing protocols and document appropriately.
  • Answer and document phone calls from patients, pharmacies, and other healthcare providers, adhering to HIPAA regulations.
  • Provide nurse visits for patients, including blood pressure checks, allergy injections, and vaccines, and reassign nurse visits to on-site providers as needed.
  • Clean exam rooms between patients and stock them consistently to maintain efficiency, quality, and safety.
  • Preload new patient clinical charts with immunizations, medications, problem lists, and past medical/social/family history.
  • Complete paperwork as needed for providers and patients, including medication prior-authorization requests and family medicine/pediatric ED recalls.
  • Perform machine/POCT controls and accept deliveries/orders, putting supplies away as necessary.
  • Document refrigerator/freezer temperatures twice daily and check expiration dates for injections, medications, samples, instruments, and supplies.
  • Sanitize instruments after use and maintain accurate and pertinent documentation in the Electronic Health Record (EHR).
  • Adhere to center infection control policies, keep work areas neat and tidy, and actively participate in Quality Assurance and Performance Improvement (QAPI) activities.
  • Participate in monthly nursing staff meetings and maintain current Relias training.
  • Adhere to all Oak Orchard policies and procedures and perform other reasonable requests from management.
Requirements
  • Ability to work with a diverse patient population.
  • Experience with EMR systems.
  • Excellent attention to detail.
  • Ability to communicate both written and verbally.
Education and Experience
  • Graduate of an accredited program for Licensed Practical Nurse with a current valid license to practice nursing in New York State.
  • One year of experience in a nursing role preferred.
