Job Summary

At Kindred Hospital Denver, we are seeking a highly skilled and compassionate Clinical Care Coordinator to join our team. As a key member of our Interdisciplinary Care Transitions team, you will play a vital role in coordinating and facilitating the care of our patients, ensuring their needs are met and their outcomes are optimized.

Key Responsibilities
  • Care Coordination
    • Collaborate with the Interdisciplinary Team and Physicians to develop and implement comprehensive care plans that meet the unique needs of each patient.
    • Monitor patients' stays to ensure effective care coordination and efficient care facilitation.
    • Stay current on reimbursement modalities, community resources, case management, psychosocial, and legal issues that impact patients and providers.
    • Refer high-risk patients who would benefit from additional support.
    • Serve as a patient advocate, enhancing a collaborative relationship to maximize patients' and families' ability to make informed decisions.
  • Discharge Planning
    • Conduct comprehensive, ongoing assessments of patients to provide timely and safe discharge planning.
    • Develop and execute effective discharge plans, utilizing critical thinking to ensure seamless transitions.
    • Coordinate and communicate with patients and families efficiently and effectively.
  • Utilization Management
    • Conduct medical necessity reviews to ensure appropriate utilization of services from admission through discharge.
    • Promote effective and efficient utilization of clinical resources.
    • Collaborate with payors to ensure timely and accurate clinical reviews, care coordination, and continued stay authorization.
Requirements
  • Education
    • Graduate of an accredited program required.
    • RN, BSN preferred.
    • OR Master of Social Work with licensure as required by state regulations.
    • OR Bachelor of Social Work with licensure as required by state regulations.
  • Licenses/Certification
    • Healthcare professional licensure required as Registered Nurse or Licensed Clinical Social Worker (LCSW) or Licensed Social Worker (LSW) if required by state regulations.
    • Certification in Case Management preferred.
  • Experience
    • Two years of experience in a healthcare setting preferred.
    • Prefer prior experience in case management, utilization review, or discharge planning.
Knowledge/Skills/Abilities/Expectations
  • Knowledge of government and non-government payor practices, regulations, standards, and reimbursement.
  • Knowledge of Medicare benefits and insurance processes and contracts.
  • Knowledge of accreditation standards and compliance requirements.
