Role Summary and Qualifications

The Transplant Clinical Nurse Coordinator (CNC) is a hybrid Charge RN/Leadership position that aims to provide high-quality, patient-centered care while overseeing departmental processes. In collaboration with other team members, the CNC directs, monitors, and evaluates nursing care in line with established policies and procedures, serves as a clinical resource for patients and bedside staff, and exemplifies a commitment to the organization's vision, mission, and values to maintain consistently excellent patient experiences and quality clinical outcomes for a specialized patient population.

Responsibilities in this Role:

  • Assist with admission and discharge processes to ensure efficient throughput and high-quality, patient-centered care.
  • Participate in the ongoing assessment of the quality of patient care services provided in the unit, in collaboration with other management team members.
  • Collaborate with staff daily to foster a positive teamwork environment that results in quality patient outcomes, service excellence, and staff satisfaction.
  • Support a patient-first philosophy and engage in routine Nurse Leader Rounding, providing service recovery when necessary.
  • Facilitate the facility's engagement improvement efforts by operationalizing current nursing strategies, including employee rounding, employee recognition, and evidence-based department process improvements.
  • Provide recommendations related to interviewing, onboarding, and training new staff. Recommend and implement actions, including training and development, conflict resolution, personnel policy compliance, performance evaluations, and/or disciplinary actions to ensure clinical excellence.
  • May assist with staff scheduling and day-to-day staffing; manage all practical aspects of staff labor in accordance with patient care needs and established productivity guidelines.
  • Support proper inventory control and assist with managing supplies and equipment in a fiscally responsible manner.
  • Permanent Charge RN position with additional office hours for leadership responsibilities.

Clinical Nurse Coordinator Qualifications:

  • Required: Previous clinical/inpatient experience with intermediate level of care patients (Two years or more).
  • Strongly Preferred: Charge Nurse experience in an inpatient setting.
  • Strongly Preferred: Previous experience with solid organ transplant patients.
  • Graduate of an accredited school of nursing. BSN strongly preferred.
  • Current licensure in the State of Colorado as a Registered Nurse, or current active multistate Registered Nurse license.
  • Basic Cardiac Life Support (BCLS) and Advanced Cardiac Life Support (ACLS) must be obtained within 30 days of employment start date.
  • Strongly preferred: Progressive Care Certified Nurse (PCCN), Certified Clinical Transplant Coordinator (CCTC), Medical Surgical Nurse – Board Certified (MEDSURG-BCTM).
