We are currently looking for a Home-Based Nurse Coordinator to serve the Denver, CO, and neighboring regions. This role is primarily home-based, requiring travel to medical facilities, clinics, and other relevant locations. The Nurse Coordinator will oversee the medical case management of work-related injuries, which encompasses assessment, planning, coordination, execution, and evaluation of care for injured or disabled individuals. This position involves collaboration with insurance providers, healthcare professionals, legal representatives, employers, and employees, ensuring close monitoring of the injured worker's progress and providing updates to employers and insurers.

Qualifications:

  • Current, unencumbered compact Registered Nurse (RN) license in Colorado
  • Bilingual (Spanish-speaking) strongly preferred
  • Experience in managing workers' compensation cases preferred
  • Certification in Case Management (CCM) preferred
  • Essential skills include organization, adherence to timelines, proficiency in technology, and strong communication abilities (written, verbal, and interpersonal)
  • Ability to work independently with minimal supervision
  • Proficient in patient education
  • Valid driver's license with a clean driving record
