We are seeking a highly skilled Nurse Practitioner to join our team at ConvenientMD. As a primary care specialist, you will play a vital role in delivering high-quality, patient-centered care to our patients.
Our Advanced Primary Care (APC) model is designed to provide a more personalized and proactive approach to healthcare. You will work collaboratively with a multidisciplinary team to deliver coordinated, efficient, and thoughtful care to our patients.
Key Responsibilities:
- Provide primary care services to patients, including routine check-ups, health screenings, and treatment of acute and chronic conditions.
- Work collaboratively with a team of healthcare professionals to develop and implement patient care plans.
- Conduct health assessments, diagnose and treat illnesses, and prescribe medications as needed.
- Communicate effectively with patients, families, and other healthcare professionals to ensure seamless care coordination.
- Participate in quality improvement initiatives to enhance patient outcomes and satisfaction.
Requirements:
- Master's degree in Nursing and certification as a Nurse Practitioner.
- Experience in primary care or urgent care settings preferred.
- Strong communication and interpersonal skills.
- Ability to work in a fast-paced environment and prioritize multiple tasks.
- Knowledge of electronic medical records and practice management systems.
