As a Nurse Practitioner, you will play a vital role in our team-based model, working closely with physicians and other healthcare professionals to provide comprehensive, family-oriented outpatient care.
The ideal candidate will have a master's degree and current New Hampshire Nurse Practitioner license. One to five years of experience as a Nurse Practitioner in a multi-disciplinary team providing primary health care to medically indigent families is preferred. BLS Certification is required.
We utilize a team-based model that includes embedded Behavioral Health Clinicians, Case Managers, Care Coordinators, and Community Health Workers. Our team is dedicated to providing integrated, affordable healthcare and education to our patients.
Key Responsibilities:
- Provide comprehensive health examinations and treatment of medical conditions
- Follow-up on problems detected within the confines of Amoskeag Health's objectives and policies
- Work collaboratively with the healthcare team to provide patient-centered care
Requirements:
- Master's degree and current New Hampshire Nurse Practitioner license
- One to five years of experience as a Nurse Practitioner in a multi-disciplinary team providing primary health care to medically indigent families (preferred)
- BLS Certification
