Care Coordinator

We are seeking a skilled Care Navigator to join our team at Eastern Connecticut Health Network (ECHN). As a key member of our care management team, you will play a critical role in ensuring seamless transitions of patient care through the healthcare system.
Key Responsibilities- Collaborate with physicians, payers, and hospital staff to coordinate care and ensure cost-effective and best patient outcomes.
- Utilize InterQual criteria to determine the appropriateness of observation or inpatient level of care and take proactive action when variances are identified.
- Provide clinical information to managed care companies in a timely manner to secure authorization for payment and prevent denials.
- Perform high-risk assessments and refer complex cases to the complex care navigator.
- Develop comprehensive discharge plans that take into account the patient's psychosocial, physical, educational, and cultural needs.
- Collaborate with the interdisciplinary team to develop and modify care plans to meet the needs of the patient.
- Current licensure as a Registered Nurse in the state of Connecticut.
- BSN preferred.
- Two to five years of experience in an RN role in a medical/surgical acute care setting.
- Additional experience in case management, home care, and utilization review/management is preferred.
- Excellent analytical, organizational, and interpersonal skills.
- Ability to communicate effectively in English, both verbally and in writing.
- Basic proficiency in personal computers.
