We are seeking a skilled Virtual Care Nurse to join our team at Eastern Connecticut Health Network. As a Virtual Care Nurse, you will provide high-quality, patient-centered care to our patients through remote monitoring and communication.
Key Responsibilities- Conduct comprehensive assessments and re-assessments of patients' medical and surgical conditions, including psychosocial, physical, educational, and cultural aspects.
- Develop and revise individualized plans of care based on patient needs and responses to treatment.
- Provide education to patients and families on pain management, medication administration, and other relevant topics.
- Collaborate with the interdisciplinary team to coordinate patient care and ensure seamless transitions.
- Document patient care and responses to treatment in a timely and accurate manner.
- Graduate of an accredited school of nursing.
- Current licensure in the state of Connecticut.
- Certification in Basic Cardiac Life Support.
- Ability to communicate effectively in English, both verbally and in writing.
- Basic computer skills.
