Job Summary

We are seeking a highly skilled Care Navigator to join our team at ECHN. As a key member of our healthcare team, you will be responsible for ensuring the efficient and effective utilization of hospital services, while promoting optimal patient outcomes.

Key Responsibilities
  • Collaborate with physicians and managed care entities to ensure care appropriateness and cost-effectiveness.
  • Conduct high-risk assessments and refer complex cases to the complex care navigator.
  • Develop and implement discharge plans that meet the patient's needs, taking into account psychosocial, physical, educational, and cultural elements.
  • Work closely with the interdisciplinary team to craft and adjust care plans.
  • Ensure compliance with InterQual criteria and reimbursement components for discharge planning in various settings.
Requirements
  • Registered Nurse license in Connecticut.
  • BSN degree preferred.
  • Two to five years of experience in an RN role within a medical/surgical acute care environment.
  • Prior exposure to case management, home care, and utilization review/management is advantageous.
Competencies
  • Strong analytical, organizational, and interpersonal skills.
  • Effective English communication, both verbally and in writing.
  • Basic computer literacy.
