Key Responsibilities:

  • Provide skilled nursing care to patients in their homes, including medication management, wound care, and vital sign monitoring.
  • Develop and implement individualized care plans in collaboration with patients, families, and healthcare teams.
  • Conduct thorough assessments of patients' physical and emotional needs, identifying areas for improvement and developing strategies for addressing them.
  • Collaborate with other healthcare professionals to ensure seamless care transitions and optimal patient outcomes.
  • Document patient progress and outcomes in a timely and accurate manner, maintaining confidentiality and adhering to HIPAA guidelines.

Requirements:

  • Current licensure as a Licensed Practical Nurse in the state of Illinois.
  • Valid American Heart Association-approved CPR certification.
