Job Overview

The Client Advocate Specialist role at Gracepoint involves providing advocacy, coordination, and continuity of evidence-based care for individuals and families in need. This position requires a high level of compassion, empathy, and effective communication skills to work collaboratively with clients and internal stakeholders.

Key Responsibilities:
  • Completion of thorough assessments and coordination of community resources based on patient needs.
  • Development of service plans, treatment plans, and/or aftercare plans with clear goals and objectives.
  • Maintenance of efficient workflow management and organization.
  • Effective communication and collaboration with internal and external resources.

Requirements:
  • Bachelor's degree in a Human Services related discipline and at least 1 year of experience in the mental health field.
  • Eligibility to apply for the Certified Behavioral Health Case Management Provisional (CBHCM-P) application through the Florida Certification Board.
  • A valid Florida Driver's License, reliable vehicle, and insurance.
