Job Summary

Avita Community Partners is seeking a skilled Case Manager to join our team. As a Case Manager, you will play a critical role in delivering support services to individuals with severe and persistent mental illness or co-occurring substance abuse issues in the community.

Key Responsibilities

  • Provide case management services to clients, including assessment, goal setting, and service coordination.
  • Develop and implement individualized treatment plans to meet the unique needs of each client.
  • Collaborate with community resources, healthcare providers, and other stakeholders to ensure comprehensive support for clients.
  • Conduct regular home visits and community outreach to engage clients and provide support.
  • Maintain accurate and timely documentation of client interactions and progress.
  • Participate in on-call rotations to provide 24-hour support to clients and team members.

Requirements

  • Bachelor's degree in Social Work, Psychology, Mental Health, or related field.
  • One year of experience working with adults with severe and persistent mental illness in a social services delivery setting.
  • Ability to work independently and as part of a team.
  • Strong communication and interpersonal skills.
  • Valid driver's license and reliable transportation.
