SOAR Case Manager

Newport County Community Health is seeking a highly motivated and compassionate SOAR Case Manager to join our team. As a key member of our community health team, you will play a crucial role in assisting clients with applying for or reinstating state-supported insurance to facilitate access to community-based behavioral health treatment.
Key Responsibilities- Assist clients in navigating and applying for state-supported insurance programs and ensure compliance with relevant federal/state laws and regulations during the application process.
- Collaborate with healthcare providers and community resources to facilitate access to behavioral health treatments.
- Maintain accurate and confidential client records in accordance with HIPAA and other regulatory standards.
- Identify applicants through team meetings, outreach, and referrals from referral sources and community partners.
- Conduct interviews to gather necessary information for SSI/SSDI applications.
- Collect medical records and other required documentation for SSI/SSDI applications.
- Write SOAR Medical Summary Reports for individual applications.
- Accompany individuals to appointments at the Social Security Administration.
- Coordinate visits to medical doctors, psychiatrists, and other specialists to obtain evidence for the application.
- Coordinate case management services with partners and provide direct case management support as needed.
- Assist the team with administrative tasks when needed.
- Bachelor's degree preferred in public health, health education, or a related field.
- Minimum of two years of experience in benefits administration, insurance coordination, or a similar role within a healthcare or social services setting.
- SOAR certification is preferred. If not currently SOAR certified, Newport County Community Health will pay for the training and certification.
- Solid understanding of public and private benefits systems and state resources.
- Familiarity with federal/state laws, accreditation standards, and regulations governing health care record confidentiality.
- Exceptional interpersonal and communication skills, both verbal and written, with the ability to interact professionally with diverse individuals.
- Compassionate, empathetic, and dedicated to delivering high-quality customer service.
- Self-motivated and capable of working independently, with strong organizational skills and the ability to manage priorities to meet deadlines.
- Possession of a valid Driver's License and reliable transportation.
