Community and Family Support Coordinator
who is passionate about making a positive difference in the lives of families and children.
Key Responsibilities:
The primary role involves utilizing advanced knowledge in social services to provide essential support and services to families enrolled in the Head Start/Early Head Start program. Responsibilities include:
1) Conducting eligibility assessments, recruitment, selection, enrollment, and monitoring attendance.
2) Facilitating parent engagement initiatives.
3) Offering comprehensive health, mental health, and nutrition support, along with resource connections.
4) Establishing partnerships with families to help them identify their goals, strengths, and necessary services.
5) Assisting in the creation and implementation of family partnership agreements that outline strategies and timelines for achieving family goals.
6) Managing and maintaining comprehensive family records.
Essential Functions:
Social Services:
- Foster collaborative relationships with parents to build trust and respect.
- Assess family needs for social services and develop tailored plans.
- Document and follow up on all referrals made to families.
- Create Family Partnership Agreements (FPA) within 45 days of enrollment, identifying strengths and needs.
- Facilitate ongoing follow-up on FPA goals.
- Provide individualized community resource information to families.
- Refer families to relevant community agencies and maintain collaborative relationships.
- Report any suspected child abuse or neglect as per established procedures.
Eligibility, Recruitment, Selection, Enrollment, Attendance (ERSEA):
- Implement and document recruitment and enrollment processes for eligible families.
- Maintain funded enrollment and address vacancies promptly.
- Verify eligibility for program enrollment accurately.
- Monitor and support family attendance, addressing any issues.
Parent Engagement:
- Organize and lead monthly parent committee meetings.
- Ensure representation at Policy Council meetings.
- Coordinate parent engagement activities through support groups.
- Develop a responsive parent engagement program that supports parental involvement.
- Promote two-way communication between parents and staff.
- Encourage parent volunteerism and participation in community services.
Health:
- Assist with child health screenings and necessary assessments.
- Promote wellness and establish medical and dental homes for children and families.
- Provide health education activities tailored to family needs.
Recordkeeping:
- Prepare transition plans for children transitioning to kindergarten.
- Document all family interactions and referrals.
- Maintain accurate and confidential records of all program activities.
School Readiness:
- Collaborate with center staff to integrate family goals with classroom efforts.
- Share relevant information with teachers to ensure coordinated services.
Program Operations:
- Participate in planning and evaluating family and community engagement objectives.
- Engage in professional development opportunities to enhance skills.
- Uphold confidentiality regarding all program participants.
Qualifications:
- Bachelor's Degree in Social Work, Human Services, or a related field.
- Minimum of two years of experience in social services, preferably with children and families.
- Certification in First Aid and CPR is required within three months of hire.
- Bilingual candidates are preferred.
Skills:
- Strong knowledge of social work principles and practices.
- Understanding of human development and available community resources.
- Proficient in data entry and family-centered practices.
