We are seeking a highly motivated and compassionate Care Coordinator - Community Navigator to join our team at Mental Health Resource Center. As a key member of our team, you will play a vital role in ensuring individuals receive the necessary services and support to successfully transition from crisis stabilization units to community-based care.
Responsibilities- Care Coordination: Serve as a point of contact for crisis stabilization units, maintain regular contact with local units to identify and assess individuals in need of services, and provide outreach and engagement to referred individuals.
- Documentation and Reporting: Ensure client records are current, accurate, and include documentation of all activities, contacts, and communications, and complete weekly outcome reports as required.
- Assessment and Monitoring: Assess and monitor for risk, symptoms of trauma, and indications of abuse, neglect, and/or abandonment, and use appropriate reporting mechanisms.
- Bachelor's Degree in Social Work or a related Human Services field from an accredited university or college.
- One year experience working in human services or mental health related field.
- Proficiency in Microsoft Office, Outlook, and use of the Internet.
- Valid Florida driver's license and insurance coverage equal to or exceeding 50,000/100,000/50,000 split limits.
