About the Role

We are seeking a highly skilled and compassionate Patient Care Manager to join our team at SouthernCare. As a key member of our hospice team, you will be responsible for ensuring the provision of high-quality care to our patients and their families.

Key Responsibilities
  • Manage the day-to-day operations of our hospice site, including administrative and clinical functions.
  • Oversee the provision of quality care to patients, including direct skilled bedside nursing care.
  • Respond to phone referrals and inquiries in a timely and professional manner.
  • Participate in on-call nursing schedule and provide direct patient care in patients' homes.
Requirements
  • Registered Nurse with a minimum of Associate's Degree in Nursing required.
  • Three years' experience in direct patient care in a clinical setting required.
  • Minimum one year of management/supervisory experience required.
  • Two or more years' experience in hospice or home care required.
  • Current license to practice nursing in the state of requested employment.
