Job Summary

We are seeking a skilled RN Admission/Discharge Coordinator to join our team at Methodist Hospitals. This role is responsible for coordinating patient admissions and discharges to ensure a smooth transition through the organization.

Key Responsibilities
  • Coordinate with bed placement to prioritize unit discharges based on daily bed needs.
  • Round with hospitalists to identify discharges and ensure timely completion of discharge medication reconciliation.
  • Provide discharge instructions to patients and/or families, and prepare patients for discharge.
  • Communicate with Emergency Department Charge Nurse to identify admissions and complete admission profiles.
  • Transfer patients to units to provide bedside handoffs and ensure patients are removed from beds when discharged.
  • Facilitate entry of rooms for EVS services and administer medications within the scope of practice and hospital policies.
  • Assess, evaluate, and document patients' physical and mental status, and identify patient care requirements.
  • Promote patients' independence by establishing patient care goals and teaching patients and significant others to understand conditions, medications, and self-care skills.
  • Assure quality of care by adhering to therapeutic standards and measuring health outcomes against patient care goals and standards.
Requirements
  • Associate's degree in Nursing required; Bachelor's degree in Nursing preferred.
  • Five years of responsible work experience as a clinical registered nurse required.
  • Current IN Licensed Registered Nurse license required; Certified Basic Life Support certification required.
