Job OverviewWe are seeking an experienced Licensed Practical Nurse (LPN) to join our team as a Clinical Care Coordinator. As an LPN at our health center, you will play a vital role in providing primary medical care to patients of all ages and backgrounds.
Responsibilities:- Deliver high-quality patient care within the scope of licensure, including patient assessment, administration of vaccinations, medications, and treatments.
- Provide patient education and support to promote healthy outcomes.
- Collaborate with the clinical team to ensure seamless patient flow and coordination of care.
- Perform point-of-care testing and document patient care in the electronic health record.
- Modify patient care plans as age appropriate and triage patients with acute symptoms.
- Request medical interpretation services as needed.
- Massachusetts LPN License.
- Current Basic Life Support (BLS) certificate.
- Bilingual in Spanish or Portuguese preferred.
- The most recent COVID-19 booster and yearly flu vaccine are required.
This is a competitive opportunity with a salary range of $28.00-30.00/Hour.
