We are seeking a skilled Care Coordination Program Manager to join our team at Advocates. This individual will be responsible for overseeing the care coordination staff, providing clinical supervision, and ensuring that high-quality care services are delivered to individuals in need.

The ideal candidate will possess a Master's degree in a related field and independent licensure (LICSW, LMHC, LMFT) preferred. They will have excellent communication skills, both verbal and written, and be able to thrive in a fast-paced, team-oriented environment.

We estimate that the salary for this position will be around $85,000 per year, depending on experience.

Key Responsibilities:

  • Oversee the care coordination staff and provide clinical supervision
  • Ensure that high-quality care services are delivered to individuals in need
  • Collaborate with the Supervisor of Clinical Services to develop and manage the program
  • Facilitate trainings and supervision for care coordination staff
  • Develop and maintain deep knowledge of services offered by the agency, affiliate agencies, and community resources
  • Provide administrative oversight of outpatient services to ensure that services and documentation meet regulatory standards and excellence in care
