The Edward M. Kennedy Community Health Center, Inc. is seeking a dedicated Licensed Practical Nurse (LPN) to join our team at our Primary Care Clinic in Framingham, MA.

About the Role:

  • As an LPN, you will deliver high-quality primary medical care to patients of all ages, races, and ethnicities.
  • You will assess, treat, and educate patients about their medical conditions, coordinate care, provide emotional support to families, and collaborate with the clinical team to ensure patient well-being.

Responsibilities:

  • Provide direct patient care within the scope of licensure, including patient assessment, administration of vaccinations, medications, and treatments.
  • Provide patient education and delegate patient care activities to ensure smooth patient flow.
  • Facilitate coordination of patient care outside the organization by managing medication, reviewing diagnostic test results, and following up on outside services provided.
  • Perform Point-of-Care (POC) testing and document patient care in the Electronic Health Record (EHR).
  • Triage patients who present with acute symptoms and request medical interpretation services as needed.
  • Collaborate with the clinical team to provide nursing-centered services through nursing visits, chronic disease management, and family planning visits.

Requirements:

  • Massachusetts LPN License
  • Current Basic Life Support (BLS) certificate
  • Bilingual in Spanish or Portuguese preferred
  • COVID-19 booster and yearly flu vaccine required
