Position Overview

The RN MDS Coordinator is responsible for overseeing and facilitating the accurate and timely completion and submission of interdisciplinary MDS Assessments, CAAs, and Care Plans in compliance with CMS RAI Manual Regulations and all relevant laws and standards set forth by Life Care.

Qualifications and Experience
  • Associate's or bachelor's degree in nursing from an accredited institution
  • Current licensure as a Registered Nurse (RN) in the applicable State, maintaining good standing throughout employment
  • A minimum of two (2) years of nursing experience, with a preference for geriatric nursing
  • Certification in Clinical Compliance (CRN C)
  • CPR certification required upon hire or to be obtained during orientation, with ongoing maintenance throughout employment
Job Responsibilities
  • Possess advanced knowledge in nursing practice
  • Make independent decisions when necessary
  • Understand and adhere to the laws, regulations, and guidelines governing post-acute care facilities
  • Implement and interpret departmental programs, goals, objectives, policies, and procedures
  • Demonstrate proficiency in all competency areas, including patient rights, safety, and sanitation
  • Maintain professional relationships with colleagues and vendors
  • Ensure confidentiality of proprietary and sensitive information
  • Comply with company policies, including those related to harassment and compliance
  • Exhibit integrity and professionalism by following Life Care's Code of Conduct and completing required compliance training
Key Duties
  • Coordinate and assist in the completion and submission of interdisciplinary MDS Assessments, CCAs, and Care Plans in accordance with CMS RAI Manual Regulations
  • Notify the Director of Nursing (DON) of any changes in a patient's condition identified through the MDS Assessment
  • Educate direct care staff on updates or changes to CMS RAI Manual or Skilled Nursing Facility Regulations affecting documentation
  • Assist in reviewing the Interdisciplinary Comprehensive Care Plan
  • Examine Final Validation Reports to ensure all assessments are completed and accepted into the CMS QIES system prior to billing, and inform the Business Office of any assessments not ready for billing
  • Analyze CMS Reports to identify late assessments and develop strategies to prevent recurrence
  • Participate in Daily PPS Meetings, Monthly Triple Checks, and other meetings as requested
  • Perform nursing duties as required
  • Provide excellent customer service and maintain a positive attitude towards patients
  • Assist in patient evacuation when necessary
  • Demonstrate reliable attendance and punctuality
  • Utilize reasoning skills and sound judgment in all situations
  • Communicate effectively and collaborate within an interdisciplinary team
  • Engage in physical activities such as sitting, standing, bending, lifting, pushing, pulling, walking, reaching, and moving throughout the workday
  • Read, write, speak, and comprehend the English language
