We are seeking a highly skilled MDS Coordinator to join our team at Frontier Management (OR). The successful candidate will be responsible for ensuring the accurate and timely completion of Minimum Data Set (MDS) assessments and care plans for our residents.
Key Responsibilities- Ensure compliance with federal and state regulations regarding the completion and coordination of the RAI process.
- Monitor MDS and care plan documentation for all residents, ensuring that documentation is present in the medical record to support MDS coding.
- Maintain current MDS status of assigned residents according to state and federal guidelines.
- Accurately complete coding of the MDS with information obtained via medical record review, observation, and interview with facility staff, residents, and family members.
- Attend interdisciplinary team meetings, quality assurance meetings, and other meetings to gather information, communicate changes, and maintain and update records.
- Assist the Director of Nursing (DON) or designee with identifying significant changes, physician orders, and verbal reports to ensure that the MDS and care plan are reflective of those changes.
- Prepare scheduling, notice of resident care planning conferences, and assist the DON in communicating outcomes and problems to responsible staff, residents, and/or responsible parties.
- Continuously update knowledge base related to data entry and computer technology.
- Complete electronic submission of required documentation to the state database and other entities per company policy.
- Ensure timely submission of MDSs to the State with proper follow-up on validation errors, maintaining validation records in a systematic and orderly fashion.
- Actively participate in the regulatory or certification survey process and the correction of deficiencies.
- Report trends from completed audits to the Quality Assurance Committee.
- Ensure the completion and timeliness of the RAI Process from the MDS through the completion of the plan of care.
- Initiate and monitor RAI process tracking, discharge/reentry, and Medicaid tracking forms through the Point Click Care system.
- Excellent knowledge of Case-Mix, the Federal Medicare PPDS process, and Medicaid reimbursement.
- Thorough understanding of the Quality indicator process, OBRA regulations, and Minimum Data Set.
- Knowledge of the care planning process.
- Experience with MDS 3.0.
- Licensed as a Registered Nurse.
