About the Role:

We are seeking a compassionate and skilled Hospice RN/Care Manager to join our team at Gentiva. As a member of our interdisciplinary group, you will provide comfort, peace, and dignity to patients and their families in their time of need.

Responsibilities:

  • Provide direct patient care services to hospice patients and supportive care to the patient's family
  • Act as primary liaison between physician, patient/family, and hospice team
  • Evaluate patient and family needs and assist in the preparation and execution of the plan of care for each patient
  • Maintain all assigned patient clinical records
  • Participate in company quality assessment and performance improvement programs

Requirements:

  • Hospice heart: compassionate, kind, and empathetic to your patients and their families
  • 1+ years of nursing experience in hospice, home health, ICU, geriatric, or oncology strongly preferred
  • RN license in the state of requested employment (or eligibility to obtain)
  • Current driver's license and automobile insurance
  • Current CPR certification
