Role Summary
We are seeking a highly motivated and compassionate Hybrid Home Health and Hospice Nurse to join our team at 3HC Home Health & Hospice Care Inc. As a key member of our interdisciplinary team, you will play a critical role in providing high-quality, patient-centered care to those in need.

Main Responsibilities
• Provide skilled nursing care to patients in their homes, focusing on comfort, dignity, and quality of life.
• Develop and implement individualized plans of care for patients, in collaboration with interdisciplinary teams.
• Conduct thorough assessments of patients' needs and preferences, ensuring that care is tailored to meet their unique requirements.
• Collaborate with other healthcare professionals to ensure seamless care transitions and optimal patient outcomes.

Requirements
• Currently licensed as a Registered Nurse (RN) in North Carolina.
• Minimum one year of experience in an acute care setting or home health/hospice environment.
• Basic computer skills and knowledge of electronic medical records.
• Strong communication and interpersonal skills, with the ability to work effectively with diverse populations.
