Compassionate Care Coordinator

We are seeking an experienced Compassionate Care Coordinator to join our team at 3HC Home Health and Hospice. As a key member of our IDG team, you will coordinate all hospice services in assigned counties, ensuring seamless care delivery and exceptional patient outcomes.
Responsibilities
- Deliver high-quality, person-centered care to patients and their families, fostering trust and rapport.
- Collaborate closely with multidisciplinary teams, including physicians, social workers, and chaplains, to develop and implement individualized care plans.
- Ensure timely and accurate documentation of patient information, including assessments, progress updates, and discharge planning.
- Communicate effectively with patients, families, and healthcare professionals, addressing concerns and resolving issues promptly.
Requirements
- Hold a valid Registered Nurse license in good standing in North Carolina.
- Prior experience in home health or hospice care preferred.
- A minimum of one year's experience in acute care settings.
- Current CPR certification.
Compensation and Benefits
This role offers a salary range of $65,000 - $85,000 annually, based on experience.
