Compensation:
- For LMSW, LAPC, AMFT: $55,000 - $60,000
- For LCSW, LPC, LMFT: $65,000 - $75,000

Overview: The Clinical Intake Coordinator for the Housing Support Program is a licensed professional (LCSW, LPC, or LMFT) in Georgia. This role is primarily responsible for conducting comprehensive bio-psychosocial evaluations, assessments, treatment planning, and delivering therapeutic services to individuals experiencing homelessness with psychiatric challenges in various community environments. The Clinical Intake Coordinator collaborates with a multidisciplinary team focused on providing field-based Medicaid-eligible housing support, case management, and clinical services aimed at facilitating the transition of homeless adults with psychiatric and co-occurring disorders into stable, supported housing, promoting independence, community integration, and long-term housing stability.

Key Responsibilities:

Leadership and Administration:
  • Develop, review, and offer guidance on program policies and protocols.
  • Participate in meetings related to Medicaid and billing, ensuring the integration of evidence-based practices and organizational values into daily operations.
  • Collaborate strategically with community organizations to enhance the care delivery system.

Direct Client Services:
  • Conduct thorough psycho-social evaluations, initial and annual diagnostic assessments, and create treatment plans while providing mental health services to community members.
  • Utilize evidence-based therapeutic approaches such as harm reduction, motivational interviewing, and trauma-informed care.
  • Maintain accurate documentation in the Electronic Health Record (EHR) system within established timelines.
  • Work cohesively with an interdisciplinary team, including case managers and peer specialists.
  • Provide crisis intervention and support as necessary.
  • Perform additional duties as assigned.

Documentation and Quality Assurance:
  • Review client charts to ensure compliance with documentation standards.
  • Supply necessary information for partner organizations to fulfill reporting requirements.
  • Coordinate with management to implement quality assurance activities that meet Medicaid standards.

Required Skills: Proven clinical experience, crisis intervention skills, and a strong commitment to the Recovery Model philosophy. Familiarity with DSM diagnoses, assessment of functioning levels, and evidence-based practices such as Motivational Interviewing, DBT, and Cognitive Behavioral Therapy. Ability to work both independently and collaboratively within a team. Proficient time management skills and strong computer literacy, including knowledge of Office 365 and EHR systems. Excellent communication skills and a collaborative mindset aligned with the mission to end homelessness.

Qualifications: A valid Georgia license as an LCSW, LPC, or LMFT is preferred. Candidates with LMSW, LAPC, or AMFT under supervision will also be considered. Experience in social services, particularly with homeless individuals facing chronic mental health issues, is advantageous. Knowledge of the Housing First model and Medicaid billing processes is essential. Must possess a valid driver's license and have reliable transportation.
