Position Summary and Responsibilities

- The Level of Care Nurse is tasked with conducting Initial and Annual Level of Care Assessments for participants in the NOW and COMP waiver programs.

- This position plays a crucial role within interdisciplinary teams, providing nursing services as required, which includes reviewing individual support plans and drafting assessments and updates.

- The role requires the administration of health risk and functional assessment tools, with training provided for these assessments.

- Responsibilities include formulating clinical recommendations and action plans that align with the personal goals of individuals while addressing their medical needs in community settings.

Key Responsibilities

- Execute initial and annual Level of Care Assessments for participants in the NOW and COMP waiver programs.

- Conduct thorough assessments of individuals' healthcare needs through various data collection methods, including history, observation, and psychosocial evaluations.

- Perform comprehensive physical and psychosocial assessments, utilizing all available information, including medical records and interviews with individuals and families.

- Carry out routine tests and monitor vital signs, ensuring accurate documentation of assessments and care plans.

- Oversee all aspects of client care, including dietary and physical activity monitoring.

- Conduct HRST assessments based on collected data and provide necessary technical assistance by observing medication administration and individual responses.

- Collaborate with healthcare team members to assess, plan, implement, and evaluate care plans for individuals.

- Deliver exceptional customer service to individuals served, their families, and caregivers.

- Analyze issues by evaluating available information and resources, developing effective solutions.

- Engage in various administrative tasks to support the Intake and Assessment Program.

- Maintain confidentiality of individuals' records and ensure compliance with established standards.

- Prepare comprehensive reports and documentation as required.

- Participate in developing individualized care plans based on assessments and treatment plans.

- Provide health education to consumers on various health topics and support staff training.

- Assess health problems and needs, develop nursing care plans, and maintain accurate medical records.

- Communicate effectively with agency personnel and other stakeholders to enhance service delivery.
