Job Summary

The Ambulatory Care Clinical Pharmacist for HIV and Infectious Diseases will provide evidence-based pharmaceutical care to patients in the ambulatory clinical setting. This role will collaborate with a multidisciplinary team to optimize clinical outcomes through medication management and patient education. The ideal candidate will have PGY-1 residency experience and a strong background in HIV and Infectious Diseases.

Responsibilities

• Provide medication management and patient education to optimize clinical outcomes
• Collaborate with a multidisciplinary team to achieve positive patient outcomes
• Participate in collaborative practice agreements to provide medication management
• Order, interpret, and monitor medication therapy-related tests
• Coordinate care and other health services for wellness and prevention of disease
• Provide education to patients and caregivers
• Document care plans in the medical record

Requirements

• PGY-1 residency experience in Ambulatory Care Clinical Pharmacy
• Strong background in HIV and Infectious Diseases
• Commonwealth of Virginia State Board of Pharmacy License required
• American Heart Association (AHA) Health Care Provider BLS certification required
• Board Certification or equivalent certification specific to clinical area highly encouraged
