About the Role

We are seeking a highly skilled Ambulatory Care Clinical Pharmacist to join our team in providing evidence-based pharmaceutical care to patients in the ambulatory clinical setting.

Responsibilities
  • Design, implement, assess, monitor, and document therapeutic plans utilizing best evidence and most effective and most economical medication treatments.
  • Collaborate with interdisciplinary teams to achieve positive patient outcomes through interactions with patients, providers, and other healthcare professionals.
  • Perform patient assessments, participate in collaborative practice agreements, provide education to patients and caregivers, and coordinate care and other health services for wellness and prevention of disease.
Requirements
  • Bachelor's in Pharmacy or Pharm.D. from an ACPE accredited College of Pharmacy.
  • PGY-1 residency or equivalent clinical experience.
  • Commonwealth of Virginia State Board of Pharmacy License required.
  • American Heart Association (AHA) Health Care Provider BLS certification required.
